The Sutter Center for Integrated Care serves to disseminate, test, validate, and continuously improve Integrated Care Management (ICM) across providers, settings and time.
Integrated Care Management (ICM) is a person-centered, evidence-based care delivery model where patient preferences, values and goals are the central drivers of the provision of care. Building on Wagner’s Care Model, ICM further defines requisite healthcare team practices and competencies needed to form collaborative partnerships with individuals, families and caregivers to fully actualize person-centered care.
Innovative leadership is essential to creating a learning organization focused on advancing the practice of patient and family centered care, with an understanding that patient engagement, supported by a health literate organization is necessary for the realization of positive health outcomes.
